Make the Diagnosis: From a Bone Marrow Transplant to a Huge Rash. What’s Your Diagnosis?

Presentation

A 49-year-old man with a history of acute myeloid leukemia who received an allogeneic bone marrow transplant 2 weeks prior developed a widespread rash. On physical examination, fever of 38.5°C (101.3°F) was present. On dermatologic examination, widespread violaceous papules and plaques were seen on the trunk and extremities. Some papules were arranged around hair follicles. The palms and soles were also involved and were painful. On histopathologic examination, vacuolar interface change involving the epidermis and adnexal epithelium and dyskeratosis in the epidermis and adnexal epithelium were seen. On laboratory examination, AST levels were elevated, and thrombocytopenia was present. Can you diagnose the patient?
